Privacy Policy

Palisades Pharmacy website (www.palisadespharmacy.ca) is owned by Palisades Pharmacy, which is a data controller of your personal data.

We have adopted this Privacy Policy, which determines how we are processing the information collected by Palisades Pharmacy, which also provides the reasons why we must collect certain personal data about you. Therefore, you must read this Privacy Policy before using Palisades Pharmacy website.

We take care of your personal data and undertake to guarantee its confidentiality and security.

What personal information do we collect?

We collect nothing about you unless you fill in a form on this site. Reading the site leaves no record that identifies you.

Through our forms we may collect:

  • Your name, phone number and email address
  • Prescription numbers, for refill requests
  • Your destination and travel dates, for travel consultations
  • Your Alberta Health Care number
  • Detailed health information in our menopause intake questionnaire — your menstrual and pregnancy history, symptoms, sexual health, personal and family medical history, medications and supplements, alcohol and tobacco use, and two standard mood and anxiety screening questionnaires (PHQ and GAD)

We do not collect payment information through this website, and we do not operate accounts or a mailing list.

How We Use Information

We use your information to:

  • Fill and manage your prescriptions
  • Assess whether a treatment is appropriate for you
  • Prepare for your consultation
  • Bill Alberta Health for publicly funded services
  • Keep the records a pharmacy is required by law to keep

We do not sell your information or use it for advertising.

Consent

Filling in a form on this site is your consent for us to use that information for the purpose the form serves. You may withdraw consent at any time by contacting our Privacy Officer, though we may then be unable to provide the service.

You are never obliged to use a form. Call us at 780-406-0808 if you would rather give us the information another way.

Personal Health Information

Most of what this site collects is health information under Alberta’s Health Information Act. The licensee of Palisades Pharmacy is the custodian of that information; our pharmacists and staff are affiliates, bound by the same duties.

The Act requires us to collect the least information necessary, with the highest degree of anonymity possible. Two examples of how that shapes this site:

  • The menopause intake questionnaire never asks your name. You are identified by a reference code we give you, and the record of which code belongs to which patient is kept in the pharmacy, not on the web server.
  • Your Alberta Health Care number is collected in a separate step and deleted within 7 days of being transcribed into your pharmacy record.

Do not use this website in an emergency. Call 911 or go to the nearest emergency department.

    Disclosure of Information

Our website may contain links to other websites that are not owned or controlled by us. Please be aware that we are not responsible for such other websites or third parties’ privacy practices. We encourage you to be aware when you leave our website and read the privacy statements of each website that may collect personal information.

How Long We Store Personal Data?

Your form answers go directly into our own database, on hosting we control. They are not sent to any third-party form or survey service.

    We may disclose health information:

    • To pharmacists and staff who need it to serve you
    • To other health providers involved in your care
    • To Alberta Health, for billing
    • Where the law requires it, or where necessary to avert a risk of serious harm

Our hosting provider, Hostinger, supplies the servers the information sits on. We do not sell health information under any circumstances.

    Website Cookies and Analytics

This site sets no cookies when you browse it, and uses no analytics or advertising trackers. Our server keeps standard technical logs, including IP addresses, for security and troubleshooting; these are not linked to your form submissions.

The intake questionnaire keeps your answers in your browser only. Nothing reaches our server until you press Submit.

All data is only accessible to our employees. Our employees are bound by strict confidentiality agreements and a breach of this agreement would result in the employee’s termination.

    Security and Retention

Form submissions are encrypted in transit. Alberta Health Care numbers are also encrypted where they are stored, with the key held separately. Access is restricted to staff granted it specifically, and our staff are bound by confidentiality agreements.

If health information is lost or improperly accessed and there is a risk of harm to you, the Health Information Act requires us to notify you, the Information and Privacy Commissioner of Alberta, and the Minister of Health as soon as practicable.

    What Are Your Rights?

You may ask to see the health information we hold about you. We will respond within 30 days.

You may ask us to correct it. Factual errors can be corrected; a professional opinion recorded at the time generally cannot. If we decline, you may attach a statement of disagreement to your record, and ask the Commissioner to review our decision.

You may also ask for a record of who we have disclosed your information to.

If you would like to know if we have collected your personal data, how we have used your personal data, if we have disclosed your personal data and to who we disclosed your personal data, or if you would like your data to be deleted or modified in any way, please contact us at: info@palisadespharmacy.ca.

    Third-Party Links

This site links to other websites we do not control. Their privacy practices are their own — please read their policies before giving them information.

    Contact Us

Privacy questions, access requests and complaints go to our Privacy Officer. We would rather hear from you first, but you may go to the Commissioner at any time without our permission.
